To help you get a better handle on all the Form´s hidden options I´ve listed them all below with the correct HTML tags.  All you have to do is cut and paste the ones you want and specify a correct value.  The only required hidden option to use the form is "recipient".

In order to use the GoldMine Web Import option you must enable it as indicated below.
